Some scenes have a spike with skulls next to their exit archways. These are not simply decorations, those skulls are "tier markers" and they indicate the difficulty of nearby enemies; the more skulls on the spike, the more challenging the enemies.[1]

This indicates that the scene is more dangerous than a typical scene of its tier, or contains pockets of higher tier opponents, and is generally designed for a group. However, the general level of enemies, resources, and quests is on par with other scenes of its tier.
