Some scenes have a spike with skulls next to their exit archways. These are not simply decorations, those skulls are "tier markers" and they indicate the difficulty of nearby enemies; the more skulls on the spike, the more challenging the enemies.[1]

For higher tier scenes, the spike may contain Daemon Skulls, which indicate two tiers each, and/or bloody skulls, which indicate four tiers each. A tier with two daemon skulls and three normal skulls is a Tier 7 scene. A tier with one daemon skull and four bloody skulls is a Tier 18 scene.
